Filing an Injured Worker’s Claim

Once a work related injury occurs:

• The injured worker is strongly encouraged to IMMEDIATELY notify their employer of injury.
• If the injury is an emergency or requires medical treatment, the employer directs the injured worker to the nearest medical facility for treatment.
• If the injury is not an emergency , the employer refers to provider directory for physician location nearby. You can locate the Interplan Provider Directory by going to www.interplanhealth.com. For additional assistance in locating a physician, please contact Sheakley UniComp, Inc. at 1-888-743-2559 or 1-513-326-8003.

Within 24 hours of seeking medical treatment, the injured worker must contact Sheakley UniComp, Inc. to report the injury or complete a BWC First Report of Injury form (FROI) and then send it to Sheakley UniComp. Please fax the FROI, with the injured worker’s signature and any medical documentation to 1-888-626-2667 or 1-513-326-8005. A FROI can also be completed on-line at www.ohiobwc.com,

Any treatment requests that fall outside of BWC’s presumptive authorization must be submitted on a C-9 Form.  Sheakley UniComp will review the request for approval.  For assistance in determining the need for submission of a C-9, please contact Sheakley UniComp, Inc. at 1-888-743-2559 or 1-513-326-8003.

If the treating physician identifies the injury requires the care of a specialist,

Sheakley UniComp requests the physician review the Interplan Health Network to locate a qualified specialist. You can access the Interplan Provider Directory by going to www.healthsmart.com/interplan.aspx. The Bureau of Workers' Compensation handles all compensation benefits. For additional assistance, the injured worker may phone BWC at 1-800-OHIOBWC..
