Group Rating Analysis
To receive a free, no-obligation analysis, simply download, print and complete an AC-3 Temporary Authorization Form (below). Sign and return via fax at (513) 326-4682 or mail it to: Sheakley UniService, Inc.,Attn: Tech Dept, One Sheakley Way, Cincinnati, OH 45242. The Bureau of Workers' Compensation requests your signature for Sheakley to perform an analysis for your company. You will receive the results of our analysis within 4-6 weeks from the time we receive your application.
